Mae y Bartneriaeth Addasiadau Bychain (PAB/ MAP) yn cyfrannu at egwyddorion Deddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) 2014 drwy hyrwyddo lles, atal, cyd-gynhyrchu, llais a rheolaeth, ac ymyrraeth gynnar. It supports individuals to remain safely in their own homes and communities, reducing the need for escalating health and care interventions. The project also contributes to the Well-being of Future Generations (Wales) Act 2015 by supporting prevention, integration, collaboration and long-term sustainability of services. The work completed can vary and include (not an exhaustive list): - Handrails (various sizes and materials) - Stair Rails (various sizes and materials) - Doorsteps - Key Safes - Bathroom adaptations
